SHELL International Trading & Shipping Company Limited (STASCO) Audit Corrective Actions Implementation

Following the successful project working with Talleres Industriales S.A (TISA) in Panama to improve diving safety and standards, Dive Safe Services® (DSS) were asked to lead on the corrective action findings produced during their recent Shell Diving Centre of Excellence (DCOE) audit conducted in March 2025.

All raised corrective actions were reviewed and a comprehensive abatement plan proposed and implemented. This involved documentation reviews and changes to some equipment to meet Shell requirements.

Project Overview

Dive Safe Services® (DSS) were first requested by Talleres Industriales S.A (TISA) in 2019 to conduct a full review of their diving division. This resulted in a six-week intensive audit resulting in a complete re-design of the diving division processes and procedures to meet with Association of Diving Contractors International (ADCI) guidance which are TISA’s chosen diving standard. This was further enhanced by implementation from UK HSE and industry best practice guidance.

Post the initial implementation DSS has visited annually conducting audits and safety training of diving personnel including but not limited to; seminars on roles and responsibilities of diving supervisors, risk assessing process and recording, emergency response drills to name but a few.

DSS has also been integral in the internal development of diving personnel through to supervisor and selection of superintendent. A comprehensive mentoring programme has successfully assessed and approved the competency of six supervisors and one superintendent developing the diving capability from two teams in 2019 to seven teams in 2026 operating from five diving support vessels.

In March 2025 Shell Diving Centre of Excellence (DCOE) visited TISA in Panama to conduct a Diving HSE Contractor Audit, sponsored by STASCO. The resultant audit findings were reviewed by DSS and TISA and an abatement plan was established lead by DSS Lead Auditor Andy Brown.

There were nine main findings. Seven were carried out remotely by way of updating documentation and driving changes to processes. Several were equipment up-grades again overseen by DSS remotely.

Site presence was conducted to verify compliance with the applicable requirements of the International Association of Oil & Gas Producers (IOGP) Diving Recommended Practice Report 411 (IOGP 411).

DSS lead the corrective action plan close out reporting and submitted all documentation to TISA for forward approval from DCOE in due course.

Key Tasks

• Lead the project to close out Shell DCOE findings;

• Review each finding and document within a corrective action tracker the steps required to close each finding;

• Identify suitable equipment and work with TISA to develop and implement IMCA DESIGN guidance; and

• Produce and deliver a seminar closing the knowledge gap ensuring all have a comprehensive understanding of IOGP 411.
